You are able to choose its template inside the page edit panel.

Basically if you have attached image on post, it will automatically pull it to use as thumb. If not, you can also define your own image by using the custom field feature, please refer to the readme.txt on how to use it.
Best of all, if you server allowed timthumb usage, you can enable it on theme option and setup its cropping method for better image display, this also apply to whether you have image attached on post or user defined image present.
Please try it out and let me know if you need more help. Thanks.
